BackgroundInternationally, patient access to notes is increasing. This has been driven by respect for patient autonomy, often recognised as a primary tenet of medical ethics: patients should be able to access their records to be fully engaged with their care. While research has been conducted on the impact of patient access to outpatient and primary care records and to patient portals, there is no such review looking at access to hospital medical records in real time, nor an ethical analysis of (...) the issues involved in such a change in process.MethodsThis study employed a systematic review framework in two stems, to integrate literature identified from two searches: Medline, CINAHL and Scopus databases were conducted, (for (1) hospitalised patients, patient access to records and its effects on communication and trust within the doctor-patient relationship; and (2) patient access to medical records and the ethical implications identified). The qualitative and quantitative results of both searches were integrated and critically analysed.Results3954 empirical and 4929 ethical studies were identified; 18 papers representing 16 studies were identified for review (12 empirical and 6 ethical). The review reveals a consensus that our current approach to giving information to patients – almost exclusively verbally – is insufficient; that patient access to notes is a welcome next step for patient-centred care, but that simply allowing full access, without explanation or summary, is also insufficient. Several ethical implications need to be considered: increased information could improve patient trust and knowledge but might transfer an (unwelcome) sense of responsibility to patients; doctors and patients have conflicting views on how much information should be shared and when; sharing written information might increase the already significant disparity in access to health care, and have unforeseen opportunity costs. The impact on medical practice of sharing notes in real time will also need to be evaluated.ConclusionsThe review presents encouraging data to support patient access to medical notes. However, sharing information is a critical part of clinical practice; changing how it is done could have significant empirical and ethical impacts; any changes should be carefully evaluated. Beads and other ‘body ornaments’ are very widespread components of the archaeological record of early modern humans (Homo sapiens). They appear first in the Middle Stone Age in Africa, and somewhat later in the Early Upper Paleolithic of Eurasia. The manufacture and use of ornaments is widely considered to be evidence for significant developments in human cognition. In our view, the appearance of these objects represents the interaction of evolved cognitive capacities with changing social and demographic conditions. Body ornamentation is (...) a medium or technology for communication, particularly of socially-relevant information. The widespread adoption of beads and other discrete objects as media for communication implies changes in the complexity and stability of social messages, as well as the scale of social networks. The relatively sudden appearance of beads in the Paleolithic archaeological record coincides with genetic and archaeological evidence for expansion of human populations. We argue that these changes reflect expanding scales of social interaction and more complex social landscapes resulting from unprecedentedly large and internally differentiated human populations. Between 300,000 and 250,000 years ago early humans in Africa and Eurasia began to use durable material substances and objects as media for signaling. Initially material signals were confined to ochre and other pigments, but over time objects such as beads were also added as technologies for sending messages. Changes in the types of materials used, their durability and costs, and the contexts of their disposal indicate a series of transitions in how early humans employed signaling media. Signaling theory from (...) biology suggests that shifts in technologies over the course of the Pleistocene reflect problems in coordinating action and resolving conflicts within increasingly large and internally differentiated societies. Social robots and their interactions with children are becoming increasingly sophisticated, with the emergence of child-robot relationships as a likely result. However, adequate measurement instruments that tap into concepts associated with child-robot relationship formation are scarce. We aimed to develop three measures that can be used to assess children’s closeness to, trust in, and perceived social support from, a social robot. We established the validity and reliability of these measures among 87 Dutch children aged 7 to 11 years old. It has been well documented that children perceive robots as social, mental, and moral others. Studies on child-robot interaction may encourage this perception of robots, first, by using a Wizard of Oz set-up and, second, by having robots engage in self-description. However, much remains unknown about the effects of transparent teleoperation and self-description on children’s perception of, and relationship formation with a robot. To address this research gap initially, we conducted an experimental study with a 2 × 2 between-subject design (...) in which 168 children aged 7–10 interacted with a Nao robot once. Transparency about the teleoperation procedure decreased children’s perceptions of the robot’s autonomy and anthropomorphism. Self-description reduced the degree to which children perceived the robot as being similar to themselves. Transparent teleoperation and self-description affected neither children’s perceptions of the robot’s animacy and social presence nor their closeness to and trust in the robot. (shrink)

This qualitative study identifies police interactions with gun violence co-victims as a crucial, overlooked component of police unresponsiveness, particularly in minority communities where perceptions of police illegitimacy and legal estrangement are relatively high. Gun violence co-victims in three cities participated in online surveys, in which they described pervasive disregard by police in the aftermath of their loved ones' shooting victimization. We build on the checklist model that has improved public safety outcomes in other complex, high-intensity professional contexts to propose a (...) checklist for police detectives to follow in the aftermath of gun violence. To build the checklist, we also reviewed the general orders of five police departments to better understand what guidance, if any, is currently given to police personnel regarding how they should interact with gun violence victims. We highlight the various domains of the flavivirus virulence factor NS1 and speculate on potential implications of the NS1 3D structure in understanding its role in flavivirus pathogenesis. Flavivirus non‐structural protein 1 (NS1) is a virulence factor with dual functions in genome replication and immune evasion. Crystal structures of NS1, combined with reconstructions from electron microscopy (EM), provide insight into the architecture of dimeric NS1 on cell membranes and the assembly of a secreted hexameric NS1‐lipid complex found in patient sera. (...) Three structural domains of NS1 likely have distinct roles in membrane association, replication complex assembly, and immune system avoidance. A conserved hydrophobic inner face is sequestered either on the membrane or in the interior of the secreted hexamer and contains regions implicated in viral replication. The exposed variable outer face is presented to cellular and secreted components of the immune system in infected patients and contains candidate regions for immune system modulation. We anticipate that knowledge of the distinct NS1 domains and assembly will lead to advances in elucidating virus‐host interactions mediated through NS1 and in dissecting the role of NS1 in viral genome replication. Evolutionary Transitions in Individuality have been responsible for the major transitions in levels of selection and individuality in natural history, such as the origins of prokaryotic and eukaryotic cells, multicellular organisms, and eusocial insects. The integrated hierarchical organization of life thereby emerged as groups of individuals repeatedly evolved into new and more complex kinds of individuals. The Social Protocell Hypothesis proposes that the integrated hierarchical organization of human culture can also be understood as the outcome of an ETI—one that produced (...) a “cultural organism” from a substrate of socially learned traditions that were contained in growing and dividing social communities. The SPH predicts that a threshold degree of evolutionary individuality would have been achieved by 2.0–2.5 Mya, followed by an increasing degree of evolutionary individuality as the ETI unfolded. We here assess the SPH by applying a battery of criteria—developed to assess evolutionary individuality in biological units—to cultural units across the evolutionary history of Homo. We find an increasing agreement with these criteria, which buttresses the claim that an ETI occurred in the cultural realm. (shrink)

En los últimas décadas se han elaborado diferentes teorías al interior de la filosofía de la ciencia que pretenden explicar cuál es la mejor manera de entender el progreso científico cognitivo. De entre ellas sobresalen por su extensión, especificidad y alcance las propuestas de Larry Laudan y de Philip Kitcher. Laudan, siguiendo a Karl Popper, Thomas S. Kuhn y retomando varias ideas de Imre Lakatos, defendió una perspectiva funcionalista a partir de la cual propuso la resolución de problemas como (...) el mejor criterio cognitivo para evaluar el progreso científico. Por su parte, Kitcher propuso como criterio del progreso científico la producción de enunciados significativos y verdaderos, defendiendo - no sin ambigüedad - un tipo de realismo científico. En este trabajo pretendo mostrar que los criterios epistémicos, a diferencia de los criterios no epistémicos, complican innecesariamente el análisis de la progresividad científica. (shrink)

Para validar su postura, Bird contrastó sus ideas con los conceptos sobre el progreso científico construidos por Thomas S. Kuhn y Larry Laudan, respectivamente. El objetivo de Bird fue mostrar que el criterio de “resolución de problemas” defendido por estos autores, es regresivo y, por ello, anti-intuitivo. En este texto analizo los argumentos de Bird en contra de estos autores y muestro en qué fallan. (...) Posteriormente se evalúa su postura a partir de sus propios fundamentos epistemológicos. “Problem-solving” as a criterion of scientific progress defended by Thomas S. Kuhn and Larry Laudan, respectively, has been criticized by several authors. Recently, Alexander Bird has suggested that problem-solving as a criterion of scientific progress is regressive and anti-intuitive. In this text I reassess Kuhn, Laudan and Bird’s positions and I show that Bird’s arguments are untenable.

Para desarrollar este trabajo, en el primer capítulo caracterizaré con cierta extensión dos de las principales posturas funcionalistas del progreso científico, a saber, la defendida por Thomas S. Kuhn y Larry Laudan, respectivamente. Posteriormente, haré un análisis crítico de estas posturas. En particular, mostraré que un modelo de progreso científico basado en la resolución de problemas no puede pasar por alto explicar cómo es que se genera un problema científico y cómo puede ser medida no sólo la importancia cognitiva (...) de los problemas científicos a resolver; sino la importancia cognitiva que tienen las soluciones a los problemas que enfrenta la ciencia. En el segundo capítulo caracterizaré y analizaré la postura realista metafísica sobre el progreso científico desarrollada por Philip Kitcher. El análisis del modelo de Kitcher es pertinente para el presente estudio debido a que este autor integró muchas de las críticas que se le hicieron a las posturas funcionalistas anteriores a su propuesta. Sin embargo, como sugeriré en la conclusión de ese capítulo, la postura defendida por Kitcher en torno al progreso cognitivo de la ciencia fluctúa entre la defensa de un realismo de corte metafísico (Kitcher 1993 y 1995) y un realismo de corte semántico (Kitcher 2001a, 2001b, 2002 y 2007) que hace difícil entender con precisión y a cabalidad su postura. Con los anteriores elementos, en el tercer capítulo revisaré el debate entre las posturas funcionalistas y las posturas realistas sobre el progreso cognitivo de la ciencia en el marco del llamado Realismo Epistemológico Convergente, con el objetivo de evaluar si otras posturas realistas pueden explicar de mejor manera el progreso cognitivo de la ciencia. Como mostraré, el realismo epistemológico convergente requiere para su defensa de una definición conceptual precisa en relación a lo que debe entenderse por “verdad” – o “aproximación a la verdad” – y su conexión con el éxito científico; así como la relación de referencia entre las entidades y organismos teóricos observables e inobservables que la ciencia postula y el mundo natural. Para analizar estas conexiones, en este capítulo revisaré varios argumentos anti-realistas en contra de la relación cognitiva que los defensores del realismo epistemológico convergente pretenden establecer entre el éxito empírico y la verdad, por un lado, y el éxito empírico y la referencia, por el otro. Posteriormente, analizaré los contra-argumentos realistas más importantes que a mi juicio se han elaborado para defender el realismo epistemológico convergente. Finalmente, mostraré que los contra-argumentos realistas no logran ser lo suficientemente sólidos para evadir con éxito los ataques anti-realistas en torno al progreso cognitivo de la ciencia. El cuarto capítulo de este trabajo lo dedicaré a caracterizar y analizar la postura fundacionista defendida por Alexander Bird. Este autor sostuvo que la ciencia progresa cuando muestra “acumulación de conocimiento justificado”. Para validar su postura, este autor contrastó sus ideas con los conceptos sobre el progreso científico construidos precisamente por Kuhn y Laudan, respectivamente. El objetivo de Bird fue mostrar que el criterio de “resolución de problemas” defendido por estos autores es regresivo y, por ello, anti-intuitivo. En este capítulo analizo los argumentos de Bird, muestro en qué fallan para, finalmente, evaluar su postura a partir de sus propios fundamentos epistemológicos. tRNAs undergo multiple conformational changes during the translation cycle that are required for tRNA translocation and proper communication between the ribosome and translation factors. Recent structural data on how destabilized tRNAs utilize the CCA‐adding enzyme to proofread themselves put a spotlight on tRNA flexibility beyond the translation cycle. In analogy to tRNA surveillance, this review finds that other processes also exploit versatile tRNA folding to achieve, amongst others, specific aminoacylation, translational regulation by riboswitches or a block of bacterial translation. tRNA (...) flexibility is thereby not restricted to the hinges utilized during translation. In contrast, the flexibility of tRNA is distributed all over its L‐shape and is actively exploited by the tRNA‐interacting partners to discriminate one tRNA from another. Since the majority of tRNA modifications also modulate tRNA flexibility it seems that cells devote enormous resources to tightly sense and regulate tRNA structure. This is likely required for error‐free protein synthesis. (shrink)

A referência central do presente artigo é a fenomenologia material da vida de Michel Henry (1922-2002), fenomenologia essa mediante a qual se evidencia o quanto Karl Marx (1818-1893) merece ser reconhecido como o pensador de uma subjectividade radical em processo de realização mediante uma praxis imanente dos indivíduos, anterior a toda a abstracção política, económica ou institucional Assim, para o autor do artigo, a aplicação desta fenomenologia à economia mostra até que ponto esta última, com as suas equivalências de mercado (...) e capital, não constitui nem uma realidade nem uma teoria autónomas. Por outro lado, o artigo salienta também a enorme importância que para as nossas vidas reais e concretas pode e deve ter uma verdadeira meta-genealogia do processo real da produção e do consumo, especialmente no contexto de uma mundialização cada vez mais objectivista, baseada na aliança entre a ciência e a técnica. Com efeito, o perigo da alienação não é permanente apenas no contexto de uma tal evolução, pois, segundo Michel Henry, o cienti-fismo moderno, ao evacuar a vida subjectiva dos indivíduos, constitui também meio propício para o desenvolvimento de um fascismo latente baseado na negação "metafísica" da individualidade, assim como na "morte" associada às abstracções vazias de uma economia que facilmente se dispensa do "factor humano" ao nível de uma racionalização metodológica sem limite. The central matter of this paper is the material phenomenology of life of Michel Henry (1922-2002), a phenomenology through which it becomes evident how much Karl Marx (1818-1893) deserves to be recognised as the thinker of a radical subjectivity in the process of realization through an immanent praxis of the individuals, prior to all political, economic or institutional abstraction. Therefore, to the paper's author, the application of this phenomenology to economics shows the measure in which the later, with its equivalences of market and capital, neither constitutes an autonomous reality nor an autonomous theory. On the other hand, the paper also underlines the enormous importance which, to our real and concrete lives, a true meta-genealogy of the real process of production and consumption can and should have, especially in the context of a more and more objectivistic globalization, based on the alliance between science and technology. Indeed, the danger of alienation is not only permanent in the context of such an evolution, then, according to Michel Henry, modern scientism, as much as it tends to empty the subjective life of the individuals, is also a propitious means to the development of a latent fascism based on the negation of the "metaphysics" of individuality and, also, on the "death" associated to the empty abstractions of an economics which easily discards the "human factor" at the level of a methodological rationalization without limit. Frequently, the social and material « control » of this contingency of our world of life is forming the exhaustive sense of that what means « Reality » Therefore, it is a critical necessity of the philosophy of religion to demonstrate that human values are including orientation to an « inconditional ». Husserl's phenomenology abandoned the fundamental phenomenological task of investigating pure appearance in favour of research into transcendent knowledge of essences. Thereby, in place of the original cogitano with its capacity for self-giving, the intentional intuition came to present itself as a mode of givenness without a radical phenomenalization of the ontologically pre-given. M. Henry, who already elaborated a „material phenomenology“ of the original hyletic self-affection as immanence of life, has demonstrated the deficiency of the phenomenological differentiation of appearing from appearance (...) by invoking four principles of phenomenology. These concern the relation between being /appearing, intuition /evidence, method /things in themselves, and reduction /givenness. Regarding this last connection J.-L. Marion's recent phenomenological approach is also worth taking into account. With the „pure appeal“, Marion at the same time questions Heidegger's priority of Dasein on behalf of the Ego. (shrink)

En toute souffrance il faut alors reconnaître cette vie transcendentalement individuée qui veut vivre son « Je-Peux » fondamental en toutes ses modalisations. Toute « thérapie » se trouve ainsi plongée dans un « pathos-avec » dont l'exercice concret se révèle dans ce « regard transcendantal » sur la souffrance d'autrui (...) sans méconnaître les possibilités de la science médicale qui reste, cependant, soumise à cette « communauté intro-pathique » primordiale. De cette manière, une certaine « praxis phénoménologique » peut affronter alors les défis de la civilisation d'aujourd'hui. Following the phenomenology of Michel Henry, this paper analyses the fundamental structure of the auto-affective life, always supporting itself for being equally co-originally the self-enjoyment. Therefore, one must acknowledge in every suffering this transcendentally individuated life which wants to live its fundamental "I can" in all its modalisations. Then, every "therapy" is plunged into a "pathos-with" whose concrete exercise reveals itself in this "transcendental view" on the other's suffering, without ignoring the possibilities of the medical science, that remains, however, subjected to this primordial "intropathic community". So, a certain "phenomenological praxis" can then face the challenges of today's civilization. (shrink)

La Déclaration commune sur la Doctrine de la Justification proposée par l'Institut pour la Recherche oecuménique de Strasbourg affirme que nous avons atteint un tel consensus concernant la question de la justification que les condamnations du XVIe siècle n'existent plus. La phénoménologie radicale de Michel Henry montre que la question de la réceptivité en tant que passivité est au cœur de toute élucidation phénoménologique dans la mesure où nous naissons dans la Vie absolue sans aucune initiative de notre part. Devant une telle situation, la réflexion discursive se montre dépourvue de ses moyens habituels, ce qui entraîne une double stratégie : accuser une telle phénoménologie radicale d'être une totalisation métaphysique ou lui reconnaître seulement une certaine valeur méthodologique, c'est-à-dire opératoire pour (...) la réduction. Notre contribution voudrait établir qu'il s'agit chaque fois, dans des discours similaires, d'une méconnaissance de la « violence » originaire que nous subissons de la part de la vie — violence qui est en même temps notre véritable Bonheur et Demeure. Michel Henry's radical phenomenology shows that receptivity is at the core of any phenomenological elucidation in so far as we are born in Absolute Life without any initiative on our part. Confronted to such a situation, discursive reflection appears to be deprived of its traditional means, which entails a twofold strategy : i.e. on the one hand, accusing such a radical phenomenology of being a metaphysical totalisation or, on the other hand, acknowledging it as a mere methodological value, i.e. an operative one towards reduction. Our contribution would thus try and set that every time, within similar discourses, what is at stake is a misconception of the primary « violence » we suffer from life – a violence which is at the same both our true Happiness and Home.
